The itinerary is sent: airplane tickets, round trip times of departure and arrival, description of the personnel providing hotel-hospital-hotel transportation, name of the assigned nurse, name of the hospital with entry and possible discharge dates, name of the treating physician, time of the programmed surgery, name of the hotel with check in and check out times, description of amenities (when applicable).
